ADMISSION
Typical indicators for Alzheimer's Care include a referral by the individual's primary care physician or a specialist in geriatric medicine, and/or a score of 24 or less on the Mini-mental exam.
In terms of the Global Deterioration Scale used by the Alzheimer's Association,, the Tred Avon program is designed for patients in Fast Stage 6 and also those in Fast Stage 4 or 5 who have non-cognitive symptoms of dementia (depression, anxiety, aggression/agitation, apathy, resistance to care, wandering/pacing, and the inability to self-regulate). The presence of such behaviors -- more than the actual degree of cognitive decline -- may indicate the need for placement in a focused program for Alzheimer's Care.
Each applicant is assessed for his or her Level of Care. Based on the information received from the primary care physician and an assessment conducted by CLC staff, a care plan is developed that is tailored to the resident's needs.
We recommend that all families plan ahead for Alzheimer's care, but we also recognize that circumstances such as an illness or injury may necessitate a streamlined approach. CLC staff work closely with residents and families to accomplish the move-in process in a smooth and timely manner.
